Confidentiality
I collect certain data from you to meet mandatory requirements regarding medical notes. I take your privacy seriously and will take all reasonable steps to ensure the protection of your data. There is a legal requirement to keep medical notes for a period of time after treatment. This can vary in length depending on your age and ability to consent but will be for a minimum of 7 years. Your details will be destroyed after this period.

As a fully accredited member of the HCPC, I adhere to their ethical framework and guidelines to ensure that you receive a professional and quality service. I adhere to current data protection legislation, including the General Data Protection Regulation (EU/2016/679) (the GDPR), the Data Protection Act 2018 and the Privacy and Electronic Communications (EC Directive) Regulations 2003. I am registered with the Information Commissioner's Office (ICO), my registration number being ZB740448. You can check their register here.

Storage of information​
Your treatment and personal information are kept securely. I take your privacy seriously and will take all reasonable steps to ensure the protection of your data.
Information that you provide by text message or Whatsapp will be stored on my password protected business phone. My email provider is Protonmail; chosen due to their robust privacy policy. Information that you provide upon booking an appointment using the Registration Form emailed to you will be collected via a secure link and stored within my electronic records management system, Writeupp. Please see their privacy policy and security information if you would like further details.

Uses
Upon referral, basic personal information will be collected for contact and identification reasons. I will use these details to communicate any changes in our appointment times or any other relevant administration changes. I will also use your details to provide you with information that will support you in using my service. During our meetings, I will take notes which will include personal and sensitive details about your life and symptoms. These notes are used solely for the delivery of a therapy service to you. To further enhance my service to you, I will also use your details to update you on any information regarding the practice.​

Your rights
You have rights relating to the information I hold to verify the accuracy or to ask for them to be supplemented, deleted, updated or corrected. You also have a right to request the transfer of your data to another individual or company. Please note that your right to be forgotten cannot override the legal requirements to keep medical notes for the mandatory periods.
You have the right to request a copy of the information that I hold about you. If you would like to have a copy of some or all of your personal information, please email me via the contact details stated above. Information will be provided to you within 30 days.

In the event of a data breach, I have a legal obligation to report this breach of your information to both yourself and the Information Commissioner's Office (ICO) within 72 hours of the breach occurring.
Sharing of data
There may be times where your information needs to be shared with third parties. I will explicitly ask your consent before doing so, and the data will be sent to third parties securely. The only exception to this is in emergency situations where you may be in immediate danger of harm and obtaining your consent would delay the appropriate support being provided to you; in such situations only sufficient information to ensure your safety will be shared with the emergency services. There are also certain circumstances under which I am required to share your personal data to comply with legal obligations, including if I am subpoenaed to court, there are safeguarding children or vulnerable adult investigations, terrorism or money laundering involved.

Information, without use of names, will be shared with my supervisor who is also HCPC accredited and regularly reviews my practice. My supervisor is also bound by a confidentiality policy. Confidentiality will be broken if I have concerns that you or anyone else is at risk. If this occurs it will be discussed in the appointment and recommendations will be discussed and documented in your notes.

If you have been recommended to see me by Quayside Crossfit, we will discuss if you would like me to work collaboratively with your personal trainers to get the most benefit within our appointments, and in your workouts.

In the event of my incapacity or death, your personal contact information will be disclosed to the executor of my Professional Will so that they can notify you and support you in the next steps. In the event of my death, following notifying you my executor will then destroy all contact information I hold on my business phone and email, and will inform Writeupp.

Consent
Use of the services of Connor Jayes trading as Atlas Physiotherapy Clinic constitutes as your approval and acceptance to the above information. You have the right to withdraw your consent at any time.
